Simple switching guide

If you need to pick iCare as your PCP

Many managed care plans require you to choose a primary doctor. We’ll help you switch over—often in one phone call.

Medicare patients

Bring or send your Medicare card. Most visits can be scheduled right away.

Managed care patients

Call your plan or log in to select iCare Medical Group as your primary care provider. If you need a provider ID, we’ll give it to you.

Steps for common plans

  • Have your member ID ready and call the number on your card.
  • Ask to change your primary care doctor to iCare Medical Group (we’ll supply any needed IDs).
  • Confirm the effective date of the change and note it down.
  • Send us a photo of your updated card so we can schedule you.
