The Google review form opens in a new tab. You can follow these steps on your phone, tablet, or computer.

Tap the blue button above. A Google window will appear. Sign in with your Google account if asked.
Choose your stars
Type a few sentences about your visit: your provider, staff kindness, wait time, or anything that stood out.
Add a short note

Add photos (optional)
If you have a picture of the clinic or materials we shared, you can attach it. This is optional.
Press Post
When it looks right, tap Post or Publish. Your review is saved to Google and shared with our team.

Create a Google account
It's free and only takes a few minutes. Visit https://accounts.google.com/signup to get started.
Open google.com and click Sign in, then Create account.
Choose For my personal use.
Enter your first/last name and birth date, then tap Next.
Pick an email address (or use your own) and set a password.
Add your phone number. Google sends a code (type it in to verify.)
Agree to the terms. You can now return here and tap Start Google Review.

No, Google reviews are public and linked to your Google account name. If you want to stay anonymous, consider providing feedback directly to the clinic via phone or email.
Yes, you can edit or delete your review at any time by going to your Google Maps profile and finding your reviews.
